Direct primary Care - $85/month

• Unlimited office visits

• Messaging and virtual visits during business hours with your provider

(Mon-Sun 8am-5pm)

• Two full lab panels per year included (CBC, CMP, Lipids, TSH, A1C: $30.77)

• Access to affordable medications through local wholesale pharmacies

Family Plans:

• Family (2 adults + 2 kids): $170/month

• Extra children: +$15/month per child

Small Business Plans:

• $50/month per employee (Essential Plan)

• Employee family members: $65/adult, $25/child

Enrollment Fee:

• $50 one-time setup fee per patient or couple/ family (waived with 6-month or annual signup)

Annual Prepay Discount:

• Pay for 11 months, get 1 month free!

Cash Pay:

• If cash is paid, must be 3 months upfront, otherwise card placed on file for renewal on 1st or 15th of every month.
